Primary Care (Dr. Logan) – Dr. Logan is trained as a primary care physician but does not provide acute care services at Sanctuary Integrative Medicine. He will work with you closely as a consultant and coach in preventive, nutritional, and functional medicine to help you address the roots of chronic health problems. He can confer with your primary care doctor if required.

Medical Records – Medical records can only be released with your authorization. A medical records release form is included with the “Consent Forms”. You are responsible for obtaining previous medical records from other physicians or health care providers. Please contact your physician or other health care provider to obtain these records. Your records should be mailed to Sanctuary Integrative Medicine, 90 Mahoney Avenue, Rutland, VT, 05701 or faxed to 802-774-5145.
